How to Access Your Medical Records

You can access your medical records online anytime through the Core Physicians Patient Portal (BILH MyChart). No need to call, fax or wait for paper records.

Your patient portal account includes test results and other details about your medical care. Through BILH MyChart, you have secure, immediate access to your medical records and can easily share information with any healthcare provider who has internet access.

To share your records, visit the Sharing Hub in BILH MyChart, available through the menu. From there, you can securely view, download and share your health information with family members, caregivers or healthcare providers when needed.

Medical records may be obtained through the patient portal at no cost.

How to Request Copies of Medical Records

Core Physicians contracts with Sharecare Health Data Services to handle all patient requests for medical record copies. If you are having difficulty obtaining medical records through the patient portal, please use the form below to request records. Completed forms must be delivered to your Core Primary Care Physician or Specialists office for validation. 

After your information is validated, we will notify Sharecare of your request and they will handle the rest and communicate with you accordingly. Once Sharecare has processed your request, they will send your records via first class U.S. Mail or via email with instructions detailing how to access the records electronically.

Cost for Copies of Medical Records

Charges for copies of medical records are governed by state and federal regulations. Sharecare calculates fees based on the HIPAA cost-based fee guidelines or applicable state law, whichever results in the lower cost. Delivery fees may also apply.

There is no charge for medical records when:

  • A Core Physicians provider refers you to another healthcare provider.
  • You request a second opinion, whether the provider is within or outside of Core Physicians.
  • You request that your records be transferred to a provider outside of Core Physicians because your Core Physicians provider is no longer employed by the practice. In this case, records will be transferred at no cost for up to six (6) months following the provider's departure.

You may be charged if:

  • You request that your records be transferred to a healthcare provider outside of Core Physicians for reasons other than those listed above.
  • You request copies of your medical records for personal use.
