Your Rights and Responsibilities as a Patient

At Core Physicians, we’re committed to providing you with respectful, transparent and compassionate care. We believe in empowering patients through clear communication, informed choices and a supportive environment. Below, you'll find an overview of your key rights and responsibilities during your time with us.

Your Patient Rights

As defined by New Hampshire law, RSA 151:21 — the state's Patients' Bill of Rights — you are entitled to:

  • Be treated with consideration, respect and full recognition of your dignity and individuality, including privacy in treatment and personal care. You also have the right to know the name, licensure status and position of all those providing your care. 
  • Be informed orally and in writing about your rights, responsibilities and facility policies — typically at or before admission — with a written acknowledgment required. For emergency admissions, information is provided as soon as feasible.
  • Receive clear information about costs — presented in writing and understandable language — regarding daily rates, what's included and excluded and services not covered by Medicaid or Medicare. 
  • Be fully informed of your medical condition, diagnostic results and treatment options, and participate in your care plan. You may refuse treatment and only participate in research with your written consent. 
  • Be transferred or discharged only after careful planning — except in specific legal circumstances — and never solely due to Medicaid eligibility. 
  • Voice concerns or grievances regarding your care or hospital policies freely and without any form of retaliation. 
  • Manage your personal finances, with optional assistance — if you authorize it in writing — that aligns with legal requirements. 
  • Be free from abuse, neglect, exploitation and any form of involuntary seclusion.
  • Be free from chemical or physical restraints, except when medically necessary and properly authorized and documented.
  • Have your medical information kept confidential, and control when and how it's released. You're entitled to a copy of your records — at a cost capped by state law — for your own use. 
  • Not be required to perform work for the hospital — unless it's therapeutic and part of a mutually agreed care plan. 
  • Communicate with, and meet privately with, anyone of your choosing — including family or resident groups. You may send and receive unopened mail and have unmonitored telephone access. 
  • Participate in social, religious or community activities that do not interfere with others' rights. 
  • Retain and use personal clothing and belongings as space permits. 
  • Receive privacy during visits, and if applicable, share a room with your spouse — unless medically contraindicated.
  • Not be denied care based on personal characteristics like race, gender identity, disability, income or sexual orientation. 
  • Be treated by your physician of choice, subject to hospital credentialing policies.
  • Have family, a caregiver or designated representative visit — particularly if you're terminally ill — unless restrictions are medically required.

Your Responsibilities as a Patient

To help us provide you and others with safe and effective care, we ask that you:

  • Participate in your care by providing accurate information about your health, including symptoms, medical history and current medications or allergies.
  • Review and follow your treatment plan. If something is unclear, or if you have concerns, please speak up.
  • Be respectful and considerate to staff, other patients and hospital property.
  • Keep appointments or notify staff promptly if rescheduling is needed.
  • Understand the financial responsibilities associated with your care and ask for help if needed.
  • Engage in your recovery by asking questions and letting us know if your health changes.
