Team-Based Integrated Care
We take a collaborative approach to pediatric health — bringing together pediatricians, behavioral health professionals and families in one supportive setting. Some Core offices include Behavioral Health Counselors (BHCs) who provide short-term therapy to help children through immediate stressors or bridge care while connecting with a long-term community therapist.
Innovative Care Model in Stratham
Our Stratham location is piloting the Collaborative Care Model (CoCM), which expands your child’s care team with a dedicated Behavioral Health Care Manager (BHCM). The BHCM works closely with you, your pediatrician, your child’s school and our consulting psychiatrist to coordinate care, monitor progress and provide recommendations for treatment or medication adjustments.

For families seeking longer-term support, our Core Pediatrics team helps connect you with trusted behavioral health resources in the community. We are committed to ensuring every child has access to the care and support they need to thrive.
Our team at Core Pediatric & Adolescent Medicine knows that your mental health is an important component of your overall health. Struggles with mental health can impact the rest of day-to-day life. Getting support for your mental health can include a variety of treatments — individual or family therapy, support groups, medication management, mindfulness practices, and in some cases more in depth programs such as intensive outpatient programs or residential treatment.
Core Pediatric & Adolescent Medicine takes an integrated, team-based approach to your overall health and wellbeing; working to provide you and your family access to the professionals needed to address all your care in one location. As we continue to expand this availability to all of pediatrics, some Core offices have Behavioral Health Counselors (BHC’s) who can provide brief, short-term therapy to individuals who may need just a few sessions to work through a current stressor, or to help bridge connection to a long term therapist in the community. Currently our Stratham location is the pilot site for a specific approach known as the Collaborative Care Model (CoCM) which adds another member to your care team, a Behavioral Health Care Manager (BHCM). They will communicate and coordinate with you, your child’s school, your primary pediatrician and our consulting psychiatrist who supports the team by making recommendations for treatment or medication adjustments. The Core Pediatrics team can also support you in connecting with resources and mental health care in our community for families who seek longer term treatment.
